An advance directive is an expression of your wishes concerning the kinds of medical care/procedures you want or dont want in the event you are unable to make medical care decisions for yourself. Your advance directive is applicable anywhere, regardless of where it is created or where you eventually may receive medical carebut only if its communicated to the institutions and caregivers who will be taking care of you when such circumstances might arise. It is not a legally binding document and its useless if its not communicated properly. The best way to be sure that your wishes will be followed is to tell your medical caregivers and your family what you want and dont wantand make sure they are all in agreement with your wishesbefore you find yourself in circumstances where others must make decisions for you.
